Refund Policy
Return Merchandise Authorization Policy. Merchandise may NOT be returned unless accompanied by a Return Merchandise Authorization (an “RMA”) issued by MPS or PuraGraft. Please contact MPS at 888-894-2487 or PuraGraft at 877-540-3258 to begin the RMA process. You may also submit an RMA by emailing MPS or Purgraft at csrteam@medicalpurcahsingsolutions.com.
Returned Merchandise to MPS or PuraGraft, without an authorized RMA number, will NOT be eligible for credit and will NOT be returned to sender. Buyer must report claims to MPS or PuraGraft promptly as designated and outlined herein. All Merchandise must be returned within 14 days of buyer receipt.
ITEMS NOT ELIGIBLE FOR RETURN:
- Any Merchandise that expires within twelve months unless Merchandise was shipped by MPS or PuraGraft to buyer with less than 6-month dating.
- Refrigerated or Hazardous Merchandise are not eligible for return.
- Schedule II (CII) Merchandise (i.e. Fentanyl, Morphine, etc.)
- Special Order Merchandise.
- Discontinued by Manufacturer Merchandise.
- Open or Partial containers from Original purchase size, including Merchandise that has been repackaged by MPS.
- Misbranded Merchandise that have been written on or misbranded in any way.
- Any Merchandise not purchased from MPS or PuraGraft.
ITEMS ELIGIBLE FOR RETURN AND CREDIT:
- Wrong Merchandise Received – Merchandise shipped in error by MPS or PuraGraft must be reported within 5 business days after receipt to receive credit. Please repackage appropriately. MPS/PuraGraft will pay for return shipping charges.
- Missing Merchandise – Shortages must be reported within 5 business days after receipt to receive credit.
- Damaged Merchandise – Damaged Merchandise must be reported within 5 business days after receipt to receive credit. REFRIGERATED Damaged Merchandise must be reported within 14 business days after receipt to receive credit. All Damaged Merchandise returned to MPS or PuraGraft will be subject to inspection and approval prior to issuing a credit. MPS/PuraGraft will pay for return shipping charges.
- MPS Repackaged Merchandise must be unopened, unadulterated and in resalable condition.
- Merchandise Ordered in Error – MPS/PuraGraft must be notified within 5 business days after receipt to receive credit. Merchandise must be returned to MPS or PuraGraft at buyer’s expense. Restocking fees apply.
- Overstock – Merchandise must be reported within 5 business days after receipt to receive credit. Please repackage appropriately. Merchandise must be returned to MPS or PuraGraft at the buyer’s expense. Restocking fees apply.
- Receipt of Short Dated Merchandise – Merchandise having less than 6 months of dating must be reported within 14 days after receipt to receive credit. Please repackage merchandise appropriately prior to shipping. MPS/PuraGraft will pay for return shipping charges.
Restocking Fee: A 25% Restocking fee, with a minimum charge of $25, applies to all returns and exchanges that are not the fault of Young Surgical/MPS/PuraGraft.
All returned merchandise must be accompanied by a signature acknowledging the product was stored and handled in the appropriate manner and with care.
